Levels Transport LLC – Terms & Conditions

 

1. Services Provided

  • Levels Transport LLC provides courier and same-day delivery services, including:

  • Medical Courier Deliveries (labs, specimens, prescriptions, equipment)

  • Same-Day Business Deliveries (documents, parcels, goods)

  • Scheduled/Recurring Routes

  • Add-on Services (temperature control, rush delivery, after-hours, additional stops, declared-value coverage)

2. Payment Terms

  • Payment is due at time of booking.

  • Only clients with a recurring service contract approved in writing by Levels Transport LLC may be invoiced weekly (Net-7).

  • All other clients must pay in full at booking to confirm service.

  • Accepted payment methods: Credit/Debit Card (Stripe), ACH, or approved company check.

  • Late payments: 5% monthly late fee will apply to unpaid balances.

3. Cancellation & Refunds

  • Standard Cancellation: Cancel more than 2 hours before pickup → Full refund.

  • Late Cancellation: Cancel within 2 hours of pickup → 50% of delivery fee charged.

  • Driver En Route / On-Site: Full delivery fee charged.

  • Refunds will be processed within 5–7 business days to the original payment method.

4. Surcharges & Fees

  • Heavy/Oversized Handling (>50 lbs): +$50

  • Excess Distance: $1.25 per mile over listed mileage brackets

  • Wait Time: $25 per 15 minutes after first 15 minutes free

  • After-Hours/Weekend: +$50

  • Temperature-Control: +$25

  • Rush/2-Hour Priority: +$30

  • Additional Insured Certificate (COI): $25

5. Liability & Declared Value

  • Standard liability: $100 per shipment unless declared-value coverage is purchased.

  • Declared-Value Coverage: $2 per $100 declared (minimum $20). Must be declared prior to pickup.

  • Levels Transport LLC is not liable for delays caused by traffic, weather, acts of God, or other circumstances beyond control.

  • Proper packaging and labeling is the client’s responsibility.

6. Confidentiality, PHI & HIPAA

  • Clients must not transmit Protected Health Information (PHI) through standard booking forms.

  • If PHI is involved, a Business Associate Agreement (BAA) must be signed before service

  • Provider will maintain confidentiality in compliance with HIPAA and all applicable laws.

7. Client Responsibilities

  • Provide accurate pickup/drop-off addresses and contacts.

  • Ensure items are properly packaged and legally permitted for transport.

  • Disclose hazardous/regulated materials.

  • Provide reasonable access for pickup and delivery.

 

8. Termination

Either party may terminate with 14 days written notice. Termination does not relieve responsibility for outstanding balances.

 

9. Governing Law

This Agreement is governed by the laws of the State of Texas.
